Formula: referral fee = the greater of (category percentage x total price) and the per-item minimum. Tiered categories apply the first rate only to the portion of the price up to the limit and the second rate to the remainder; threshold categories apply one rate to the whole price depending on which side of the limit it falls. The total price is item price plus delivery and gift-wrap charges.
The VAT point: UK shelf prices include VAT, and Amazon applies the percentage to the total the buyer pays. Entering an ex-VAT price understates the fee; the VAT switch grosses it up for you.
Simplification: Clothing and Accessories uses the total price for its bands; the page words the bands as "item price".

Is the Amazon UK referral fee calculated on the price with or without VAT?
Amazon's UK pricing page says the referral fee is a percentage of the total sales price, defined as the total amount paid by the buyer including delivery and gift wrapping. UK consumer prices include VAT, so the fee base is the VAT-inclusive price, not the ex-VAT price. The page does not use the words "VAT-inclusive", so confirm against a settlement report for your own account.
What is the minimum referral fee on Amazon UK?
Most categories carry a per-item minimum of £0.25; the fee is the greater of the percentage and the minimum. Books, Music, Video and DVD, Software, Video Games, Video Game Consoles and Grocery and Gourmet are listed as not applicable.
Do the referral fee and the selling plan fee stack?
Yes. Amazon states that referral fees are in addition to selling plan fees: £0.75 per item on the Individual plan, or £25 a month excluding VAT on the Professional plan.
How often do the rates change?
