What the ministry page says

Reinstating a suspended Ontario driver’s licence costs $281; the fee does not apply if the suspension was for a medical reason. From outside Ontario you can pay by cheque or money order drawn on a Canadian bank, or by Visa or Mastercard with a signed authorization letter. Written and road test fees, if you need to take them, are extra, and re-examination fees are payable at a DriveTest Centre.

For a fully licensed driver: invalid 1–3 years needs an eye test and you receive a temporary licence valid 60 days to finish the re-examination; 3–10 years needs vision screening, a knowledge test and the G1 and G2 exit road tests with no waiting periods; 10 or more years means re-applying as a new driver and serving all mandatory waiting periods. Novice drivers invalid more than 3 years serve the mandatory waiting periods.

Other costs

Ontario’s fee page also lists $35.75 to replace a driver’s licence and $90 to renew one. See the Ontario fee lookup. Other provinces set their own reinstatement fees and are not covered here.
