Quick answer
The U.S. federal definition of a credit hour is at least one hour of class and a minimum of two hours of out-of-class work each week, over about 15 weeks for a semester credit. For 15 credits that is 15 hours in class plus at least 30 hours of study, about 45 hours a week.
Worked example
15 semester credits over 15 weeks: class time 15 x 1 = 15 hours a week; out-of-class work 15 x 2 = 30 hours a week; together 45 hours a week, or 675 hours across the term. With 12 credits, a 12-week term and a 15-hour job: 12 + 24 = 36 hours of school a week, 51 hours with the job, and 432 school hours in the term.
How it works
In class = credits x 1 hour. Out of class = credits x hours per credit (2 by default). School hours per week = the two added. Term total = school hours per week x weeks. The per-day figures divide the weekly school hours by 5 or 7.
The wording comes from 34 CFR 600.2, which defines a credit hour as an amount of student work defined by an institution and consistent with commonly accepted practice in postsecondary education, that reasonably approximates not less than one hour of classroom or direct faculty instruction and a minimum of two hours of out-of-class student work each week for approximately fifteen weeks for one semester or trimester hour of credit, or ten to twelve weeks for one quarter hour of credit, or an equivalent amount of work. Labs, internships and studios can be counted differently under the same definition.
This is a federal minimum used for credit-hour rules, not a promise of grades or a rule every school follows. Canadian and other institutions define credits differently, so check your own calendar.

FAQ
How many hours should I study per credit hour?
The U.S. federal credit-hour definition uses a minimum of two hours of out-of-class work for each hour in class, each week. That is a minimum used to define a credit, not a personal prescription.
How many hours a week is a 15-credit course load?
By that definition 15 hours in class plus at least 30 hours of out-of-class work, so about 45 hours a week.
Does it apply to quarter credits?
The definition uses ten to twelve weeks for one quarter hour of credit with the same weekly hours. Set the weeks box to your term length.
Does this apply in Canada?
The source is a U.S. federal regulation. Canadian institutions set their own credit definitions, so use this as a reference and check your calendar.
